What is actually going on
Night sweats and hot flushes share the same hormonal mechanism, but night sweats carry an additional layer of disruption that hot flushes during the day do not. Sleep is structured in cycles of light sleep, deep sleep, and REM sleep.

What actually helps
The most effective approaches address the underlying hormonal disruption rather than just managing the sweat itself.
HRT is the most effective treatment for night sweats, addressing the oestrogen withdrawal that narrows the temperature tolerance zone and triggers the flush response. Most women notice significant improvement in frequency and severity within weeks. For women already on HRT for other symptoms, night sweats are likely to ease as part of the broader hormonal stabilisation.
Micronized progesterone taken at bedtime deserves specific attention. A meta-analysis of randomised controlled trials found that micronized progesterone at 300mg at bedtime improved sleep onset latency. Because progesterone supports GABA activity in the brain, it addresses the sleep disruption at the neurological level rather than just reducing the vasomotor response. When to see a doctor
Night sweats that are occurring most nights, significantly disrupting sleep, leaving you too fatigued to function during the day, or that are accompanied by other unexplained symptoms warrant clinical assessment. Night sweats can occasionally have causes other than perimenopause, including thyroid dysfunction, certain infections, and some medications.
